Vecuronium bromide is an FDA-approved peripherally acting, monoquarternary, steroidal, non-depolarizing neuromuscular blocker with an intermediate duration of action used during general anesthesia to facilitate endotracheal intubation, to aid in surgical relaxation, and, less commonly, in the intensive care setting to units.
It has few cardiovascular side effects and is cleared rapidly by biliary excretion. Thus, it is safer than pancuronium in the presence of renal failure. Interference with excretion or potentiation of effect has been suggested when vecuronium is used in combination with metronidazole, aminoglycosides, and hydantoins.
The principal pharmacologic effects demonstrated by vecuronium revolve around its competitive binding of cholinergic receptors located at motor end plates. This competitive binding results in muscle relaxant effects that are typically employed as an adjunct to general anesthesia.
